Draw Request Notification E-mail Form

ALL DRAW REQUESTS/PAPERWORK MUST BE SUBMITTED TO THE LOCAL TITLE COMPANY. By completing this form, you are “informing” CLC that you have submitted a draw to the local title company.

In most cases, the Sworn Statement and appropriate lien waivers must be completed and turned into the local title company (where you closed your construction loan) for the disbursement of funds to be approved. If you have completed your Sworn Statement on your personal computer and have it saved as a Word or Excel Document you should contact your local title company (where you closed on your construction loan) and inquire if emailing options are available for requesting a draw. Title companies may vary on policies and procedures. If emailing options are available, you may be able to simply “attach” your Sworn Statement with this Draw Request Notification and send it to the local title company and CLC simultaneously.
