Draw Request Notification Form

In order to request a draw disbursement from your construction loan, please complete the following procedures:

  1. Complete the appropriate paperwork (Sworn Statement and Lien Waivers) and submit this information to your title company (where you closed on your construction loan).

    Note: For help in completing the Sworn Statement and/or Lien Waivers Forms, see Rapid Draw Forms under the Forms & Tools section of this website. Draw procedures/policies may vary among title companies. You should receive draw instructions from the title company upon closing of your construction loan. You may also contact the title company for instructions.

  2. Notify CLC of your draw request by completing and e-mailing this form.

Please allow time for any necessary inspections that may be required before funds will be disbursed.
